  History & Consent forms

There are mandatory forms attached to each appointment. Please fill out the forms specific to your service(s). Every new client must fill out the appropriate forms  advised in the service description prior to your initial service. Failure to do so prior to your appointment will take away from the allotted service time,

as these forms are mandatory before starting treatment. 

Appointments & Deposits

All initial facial appointments are paired with a 10-15 minute consultation. Again, please make sure to have your forms completed. Please follow the recommendations attached to your service in its description.  

 

Please note all services are subject to a 50% deposits. This deposit secures your appointment and will be added towards your balance due at the end of your service.

 

Cancelations

All cancellations must be made within a 24hr window. Failure to do so will result in loss of your deposits. Rescheduling is okay and allowed no more than 1 per appointment.

Your appointment will be considered missed after 10 minutes. 

Missed appointments/ no call no show appointments will be charged 100% of the remaining balance.

You will not be allowed to rebook unless that remainder is paid.
